Supporting Documentation

What information do we need from you?

To assist you to establish and manage your salary packaging payments effectively, you will be required to provide certain documentation.

We call this ‘supporting documentation’ and we will ask that:

  • Appropriate supporting documentation provided supports the nature, type and classification of the payment being made on your behalf
  • Links the claim to you

Supporting documentation required will vary based on the type of benefits included in your salary packaging arrangement.

General Living Expenses

For reimbursement payments, we will require proof of two consecutive payments made by you.

Home mortgage repayments (excluding investment properties) Mortgage statement showing name, address, account number and balance of debt.

RentResidential Tenancy Agreement together with payment details.
Personal or car loan repayments Loan statement showing name, address, account number and balance of debt.
Private health insurancePolicy document showing policy membership number and payment details.
Salary Packaging and Employee Benefits Card With your card application, you will need to provide proof of identity. This can be the document number from either your Passport, Drivers Licence or Proof of Age Card. If your Passport is not Australian, you will need to also provide the country of issue.
School fees or student fees Invoice, and if you prefer reimbursement, proof of payment.
Council rates Invoice, and if you prefer reimbursement, proof of payment.
Direct credit card payments Credit card statement showing name, address, account number and balance of debt which must be provided monthly.

Entertainment Benefits

Dine-in mealsYour bank details for the reimbursement.
An invoice or receipt with:
- The name of the restaurant or café
- The amount paid
- Date of purchase.
Holiday Accommodation Your bank details for the reimbursement.
An invoice or receipt with:
- Your name
- Name of supplier
- Cost of purchase
- Amount paid / proof of payment
- Date of purchase or,
- Date range
- Description of goods purchased
- Identifies itself as being for your exclusive use

A Pre–Packaged Holiday also requires a brochure to promote holiday as a package
Taxis and holiday car hireYour bank details for the reimbursement.
An invoice or receipt with:
- Name of supplier
- Amount paid
- Date of purchase
